IMPORTANT DETAILS: Spots can go quickly ‐ Register early! The total cost for the weekend is $200. Complete and mail this form with a non‐refundable check for $75, and you will receive written confirmation after your reservation is processed. The balance of $125 is due 30 days before your Walk begins. If the weekend is full, you will be placed on the waiting list. *Full weekend payment of $200 will be non‐refundable 14 days prior to the start of the Walk weekend. *Your spot is reserved only after the DW registrar receives this form and the non‐refundable deposit of $75. Fill out this form completely, and make your non‐refundable deposit check of $75 payable to “Crossroads Church.” Please print “Discipleship Walk” on the memo line.
